Campus Health Center
The Campus Health Center provides affordable and accessible health care to UHMC students, faculty
and staff. The facility is managed by Denise L. Cohen, APRN, a Nurse Practitioner, with emphasis on
health promotion. HMSA, HMAA, UHA and most other insurances are accepted. The center is unable to
accept Kaiser or other HMO insurances.

The Campus Health Center is an approved family planning facility for the state Department of Health,
offering low-cost pap tests, birth control options information and administration, pregnancy
testing, and prevention and treatment of sexually transmitted diseases. Also provided are physical
examinations for school, sports, Department of Transportation, Coast Guard, and others, as well as
vaccinations, TB testing and clearance, and smoking cessation programs. Preference is give to those
with appointments. Call 808-984-3493.
The University of Hawaii Maui College Campus Health Center provides:
- TB Clinic: Mon, Tues, Wed, Fri from 9 - 4 pm
or by appointment (except Thursdays)

All applicants must follow Hawai’i Administrative Rules Title 11 Department of
Health:
Submit to the Campus Health Center a current (within 1-year of start of class) the health
requirements below. You may not register for classes until you have
received health clearance.
Health Clearance Requirements are:
Comply with State of Hawai'i tuberculosis clearance guidelines
ALL students enrolled at Maui College must have tuberculosis (TB) clearance. A negative TB skin
test (PPD - Mantoux) within one year prior to start of class your enrollment is required. Skin
test results must be read in 48 to 72 hours. If positive, a chest x-ray is required. TB tests
performed outside the United States, or skin testing using methods/materials other than mantoux
will not be accepted. Once you have met your TB clearance requirement, your clearance is valid
for four years of continuous enrollment.
Provide documentation of immunity to measles, mumps, and rubella
Two doses of measles vaccine are required, with at least one of the two being an MMR (Measles,
Mumps, Rubella). First dose must have been given as of January 1, 1968, AND on or after the
first birthday. A second dose must have been given at least 4 weeks after the first dose.
Measles, Mumps, and Rubella immunizations may be waived if: 1) Student was born before 1957, 2)
Serologic evidence of immunity to measles, mumps, and rubella, or 3) A medical exemption may be
granted only by a U.S. licensed physician (M.D. or D.O.) must state in writing giving a specific
vaccine would endanger the student’s life or health.
Note: Although not required for enrollment, it is highly recommended that students should
also receive the following immunizations: a) Hepatitis A and B, b) Tetanus/Diphtheria, c) Polio
and d) Meningococcal. We especially urge students who intend to live in the residence halls to
consider the Meningococcal vaccination, as there is an increased risk of this
highly contagious disease in this campus population.
Persons claiming religious exemption to immunization requirements must sign a waiver form and
provide a letter claiming the exemption status from a recognized religious organization.
